Please follow the steps below to schedule your appointments and get started. Failure to provide the requested documents may result in a delay in starting your program.
Step 1
Fill out the “Get Started” form by clicking the button below and providing the required documents.
If you prefer an encrypted form of communication with the office rather than standard SMS/email, you may download the Spruce Health App.”
Step 2
Schedule your 30-minute onboarding call with the office staff by clicking the “Schedule Onboarding Call” button below and selecting an available time. Your program, next steps and answers to any of your questions will be reviewed with you during this call.
Step 3
If you are interested in a Continuous Glucose Monitor, you can download the appropriate app below.
Enter your email address below to start gaining premium access to the Toward Health App * download the app & create an account using the same email address you used to enroll.
*If you’ve already created an account & have subscribed to premium access, you must cancel your subscription.
Step 4
Using a computer or laptop, please navigate to the Onboarding course by clicking below. This is a REQUIRED step to get your program started.
